Designate an Agent for Third Party Ballot Delivery
If a voter is unable to personally apply for or deliver their mail ballot, they may designate an agent to help. The voter must designate someone in writing, and that agent must sign in agreement, using this official Pennsylvania form: Authorize a Designated Agent Form. The agent must present (or enclose) the completed form when handling the voter’s ballot materials.
